As reported, the 77-year-old man was rushed to the hospital by his family members due to severe left chest pain, sweating, convulsions and fainting, shortness of breath, low blood pressure, slow heart rate and SPO2 85%.

About 3-5 days before the hospitalisation, the patient had showed signs of fatigue and chest tightness. In addition, the patient has a history of hypertension.

Immediately upon admission into the hospital’s Emergency Department, the patient suffered from respiratory failure, hypotension and he was in desperate need of oxygen and infusion of vasopressors.

Also unfortunately, the patient was confirmed to contract COVID-19 via a mandatory rapid antigen test at the hospital.

The factors conducted coronary intervention procedure for this coronavirus –infected patient. They performed resuscitation and coronary angiography. They also detected that the patient had complete occlusion of the right coronary artery with main coronary artery stenosis and left anterior interventricular artery.

After nearly 2 hours of intervention, resuscitation and close monitoring, the patient's chest pain was relieved and his blood pressure was improved.

According to Doctor Ho Van Phuoc, Head of the Department of Interventional Cardiology of the Da Nang General Hospital, this was a special case because the patient both contracted COVID-19 and suffered an acute myocardial infarction, causing a drop in blood pressure, cardiogenic shock, a high risk of heart failure. The mortality may be higher if not recognized early and managed promptly.

Such an emergency case requires rapid revascularization during the golden time for the patient, while ensuring the practice of safety measures against COVID-19.

The patient has been transferred to the Department of Tropical Medicine for further COVID-19 treatment and cardiovascular monitoring.
